How do I become a telehealth patient?
Step One: Be Teachable, Resourceful, and Committed
We ask you to follow directions on our website before scheduling an appointment. Why?
If you are unwilling to learn new information, watch a short video, or read a few pages, you will simply not get well. The only possible way that we cannot help you is if you first decide not to allow us.
Be teachable:

We interview and select only those who are teachable, resourceful, and committed. If these words describe you, welcome home.
Step Two: Watch and Read
We ask that all prospective patients watch our office tour and read the frequently asked questions section of our website, as well as the telehealth specific FAQs at the bottom of this page before scheduling an interview.
These steps must be completed before speaking with our office. If you call our office and have not done so, you will be directed back to this page.
Step Three: Schedule Your Interview
When you have completed these instructions and are ready to schedule your interview, call us at (423) 800-6800 or text us at (423) 225-5060. After answering a few questions, you will select a time for your 30 minute interview.
NOTE: You will find a phone code while completing step three. Please have this code ready when you call or text our office. It is required that you provide it in order to schedule your interview.
Step Four: Complete Paperwork
Download and complete the required paperwork in the comfort of your own home.
It is vital that all sections be completed. Mark “N/A” for any sections that are not applicable to you so that we know the item was not overlooked. If any sections are left blank, your paperwork will be returned to you for completion. Email these forms to our office 48 hours before your interview. If you are unable to print these forms, we can mail them to you.
Step Five: Explore resources

Step Six: Interview
During this 30 minute interview, we will determine whether or not you are eligible to be a patient. If so, you will then be able to get started on our telehealth program.

What does this program include?
Here is a brief overview of our four month telehealth program:
- Weekly Food Coaching
- 16 weeks of realistic and attainable step-by-step changes to create lifelong habits.
- Divine Design Academy
- A four module educational program consisting of 32 Wellness Workshop videos with quizzes.
- Read Food Made Simple
- This cookbook makes food simple by taking it back to the basics. It includes a real food guide and 80 recipes to get you started on your healthy eating journey.
- Core Four Supplement Package
- Four whole food supplement essentials.
- Whole Food Proteins
- Two whole food proteins to begin implementing vegetable protein shakes.
- Whole Food Supplements
- Four months of whole food supplements based on symptoms and needs.

- Lists of vetted recommendations for healthy products and services.
Do I need to be in-person for any part of this program?
No, but it is very beneficial to come in-person for detoxification and chiropractic at least once during the 16 weeks.
How long is the weekly nutrition coaching? What does it typically involve?
The weekly coaching is done entirely through text. You will send us your weekly food logs, and we will send back recommendations and goals. You will also have access to texting us questions as needed. We’ll do a progress check-in every four weeks. You will also have a video call with Dr. Spradlin at 8 weeks and at the end of the 16 weeks.
How much time per week should participants expect to spend on the program?
Around 2-3 hours per week. The majority of that will be the Divine Design Academy videos and quizzes.
What happens after the 16 weeks? Any follow-up care?
We recommend a quarterly check-in where we will review your symptoms, have Dr. Spradlin review your progress, and update supplement recommendations. We would then ship your supplements for the next three months. We would recommend in-person detoxification visits as often as you are able to travel to Cleveland. Additionally, once dietary goals have been met, we will do a 21 day purification program with you.
